Purpose Driven Home Health Diabetes Care Program focuses on the education,
monitoring, and treatment of patients with Type 1 and Type II Diabetes. Diabetes can be
a debilitating disease if left untreated or is poorly treated. It puts individuals at risk for
heart disease, stroke, high blood pressure, kidney disease, neuropathy, wounds, and
Our Diabetes Care Program provides skilled care by registered nurses and a
multidisciplinary team under the orders of the patient’s physician to provide medical
treatment to promote positive health outcomes as well as an improved quality of life. We
provide patient and caregiver education, assessment, monitoring, client education
materials, and collaboration with the patient's physician with special respect to the
prevention of complications associated with diabetes.
Patients who would benefit from our service are those with multiple hospital admissions,
diabetic wounds or foot ulcers, multiple new medications, patients new to insulin
therapy, or underlying disease processes that may contribute to poor diabetic control.

Client education and self-monitoring
Foot assessment with follow-up education
The goal of is to enhance the well-being of our patients by reducing the number and
frequency of hospital admissions and visits to the emergency department, as well as,
diminishing the complications and risk factors associated with diabetes.
